You can register with the practice if you live within our boundary area. This covers parts of postcodes NE31 and NE32 which include parts of Hebburn and Jarrow. If you aren’t sure if your address is covered please ask at reception.
If you register with Ellison View Surgery you must complete a GMS1 form and must have a health check before you become a registered patient. If you do not attend the surgery for your new patient health check, you will not be able to register with the Practice.

Once you have registered and had your initial health check you do not have to see the GP you are registered with but can choose any of the GPs at the practice.
