How To Register As A New Patient
To register as a patient at Aeti Surgery, your home address must be in our catchment area. Our catchment area does vary depending on which surgery you wish to register with.
Steps to Register:
- Download and print the GMS1 form and a new patient form.
- Fill them out.
- Then, bring them along with:
- Proof of Identity: A photo ID like a driving license or passport.
- Proof of Address: A utility bill or bank statement.
- NHS Number: If you’ve been with another GP or received NHS care, you likely have one.
For children, include vaccination records from abroad if it’s their first registration.
Forms to Download:
- GMS1 new – Complete page 2 for all applicants.
- New Patient Registration Form-Adult v14.2 082019 – Fill in BLOCK CAPITALS. One form per family member, plus GMS1.
- New Child Patient Registration Form 0 – 16 year’s old – Also in BLOCK CAPITALS. Include a passport or birth certificate, plus GMS1. For newborns, use the FP58 form.
For newborns, parents can register their baby with the FP58 form, usually given with the birth certificate.
Registration can take up to 7 days. You cannot book an appointment until it is complete. If you need urgent care, call NHS 111.
You will also fill out a questionnaire. This helps us understand your medical history before records arrive.
Please bring all forms and documents in person. For checks, visit after 2pm, Monday to Friday.
You can begin the registration process by using the new Register with a GP online service. You will still need to visit the GP surgery in person with your ID.
Every new patient gets a “summary care record.” More information about SCR and expressing your choices can be found at: NHS England Summary Care
Patients are reminded that registering with a GP surgery does not automatically entitle you to free NHS hospital treatment.

- Current passport (includes nationality, birth details, passport number, expiry, photo, signature).
- UK driving licence (photo card with signature or old style).
- Provisional UK driving licence (photo-card).
- Northern Ireland Voter’s Card (shows current address).
- Armed Forces ID Card.
- Residence permit (for non-EU nationals, issued by Home Office).
- Firearms or shotgun licence.
- HM Revenue & Customs tax document (includes name and address, no P45s or P60s).
- Benefits agency letter (confirms eligibility for benefits or pension).
Proof of address: list of acceptable documents dated within the last 3 months
- Utility bill (gas, electric, water, or landline; must include name and address).
- UK driving licence (photo-card with signature or old style; provisional not accepted).
- Bank, building society, or credit card statement (must include name and address).
- Current year local authority tax bill (e.g., council tax).
- Local authority rent book.
- A solicitor’s letter (confirms house purchase) or land registry (proof of previous address) are needed.
- HM Revenue & Customs tax document (includes name and address; no P45s or P60s).
- Benefits agency letter (confirms eligibility for benefits or pension).
